Move Out Form

When you are ready to move out you need to comply with the following conditions and fill out the form below.

1. Give the owner 15 days notice prior to the end of the month upon which you plan to vacate the unit(s) by submitting the following form or the offline version at the bottom of the page.
2. Once the unit is empty:
   • Sweep the unit clean.
   • Remove your lock from the door.
   • Call 435-586-9151 or use the contact form on this website to notify us that you are out of the unit.
3. Pick up deposit two weeks after move-out at our office (415 North Main Street, Suite 303, Cedar City Utah, UT 84721).

   When using this Service, you agree to transact business using electronic communications, electronic records, and optionally electronic signatures rather than using paper documents.
